The next step for all new clients is a free home health care consultation.  We will schedule a time that is convenient to visit you in your home. Our Care Manager will review your medical history and perform an evaluation of your physical, emotional, spiritual, and social needs.  This one hour meeting also helps us to “match” the right caregiver with you or your family member. 

The in-home consultation provides us with the information required to create a Care Plan, which will be used to document and track our client’s status and acts as an action plan for our elderly care.

The consultation will address:

•    Medical review

•    A home safety check

•    Mobility status

•    Mental status

•    Social and recreation needs

•    Your caregiver “match”
