Upon completion of the health care assessment and review of our options, our Care Manager will complete a detailed home health Care Plan which will address issues such as:

• Action plan for our caregivers or nurses to follow

• All medical and social information

• The frequency and duration of caregiver visits appropriate for each client

• A plan for individualized meal preparation

• Assistance the client may require to maintain and/or increase social  interaction

• Consideration of additional services that could help the client feel more comfortable, happier, safer and ultimately more independent 

• The right caregiver match

When the care plan is complete we can begin our home health care service. Whether you need 2 hours per week or 24 hour elderly home health care, we will start our services as soon as you need them.  When we leave your home after the initial consultation, we will try to match a caregiver to your health, personality and schedule needs.  Once we find a match, we will call you with a profile of our caregiver and a confirmed start date and time. 

For back up care and emergency care, we will strive to have your service start within 4 hours.  An assessment will be completed as soon as we can after starting service.  (Subject to location.  Please call for details.)
